White particles at the base of the glans may be caused by ectopic sebaceous glands or pearly rash, and most are benign lesions. The main causes include ectopic sebaceous glands, pearl rash, poor local hygiene, fungal infection, condyloma acuminata and other causes.
1. Ectopia of sebaceous glands
Sebaceous heterotopia is the hyperplasia of sebaceous glands in atypical locations, manifesting as white or light yellow papules ranging from pintip to millet size, which are painless and itchy. Commonly found on the inside of the foreskin, the glans coronal sulcus, etc. This phenomenon is related to changes in hormone levels and does not require special treatment. The area needs to be kept clean and dry daily to avoid excessive friction and stimulation.
2. Pearl rash
Pearly papules, also known as penile pearly papules, are most common on the edge of the coronal sulcus of the glans penis and are arranged in a white or translucent granular pattern. It is a physiological variation and may be related to local irritation or excessive foreskin. It is usually not contagious or malignant. If it affects the appearance, laser treatment can be considered. It is recommended to wear loose cotton underwear every day.
3. Poor local hygiene
Not washing for a long time may lead to the accumulation of cutin or smegma, forming a white granular substance. When accompanied by slight odor or itching, the perineum needs to be washed with warm water every day and avoid using alkaline lotions. If combined with redness, swelling and pain, infection must be ruled out.
4. Fungal infection
Candida infection may cause a white, fluffy discharge or dot-like white spots on the glans penis, along with severe itching and erythema. After medical diagnosis, you need to use miconazole nitrate cream, clotrimazole ointment and other antifungal drugs. Your spouse needs to be treated simultaneously to prevent cross-infection.
5. Genital warts
Sexually transmitted diseases caused by human papillomavirus infection initially appear as small, light red papules, which can fuse into cauliflower-like growths in the later stages. It is contagious and requires timely cryotherapy or podophyllotoxin tincture to remove warts. Sexual intercourse is prohibited during treatment.
You should pay attention to the perineal breathability on a daily basis and avoid sitting in a humid environment for a long time. When changes such as enlarged particles, ulcerated exudate, and basal sclerosis occur, you need to immediately visit a dermatology and venereology department or urology department to rule out malignant lesions. It is not recommended to squeeze or apply folk remedies on your own. Wrong handling may lead to secondary infection or aggravation of the condition.
